Welcome to the First Judicial District Court’s Forms Section

The New Mexico Supreme Court has approved certain forms that can be used in district courts throughout the state, if you do not have an attorney and are representing yourself.  These forms are avaliable on the  New Mexico Supreme Court’s website, and at the Self-Help Center of the First Judicial District Court.

In addition, the First Judicial District Court provides a variety of individual forms and packets designed specifically for use in various situations and case types within the court’s district. Please be aware that they do not contain packets and forms for every situation, such as a civil complaint, but several packets do contain the forms required to begin a case, with some also including the forms required to finish that case. The cost of each packet varies from $5 to $30 per packet, with many individual forms being free.
