Forms

You can get forms from a number of sources – on-line, books, the Supreme Court law library.

The New Mexico Supreme Court has adopted forms for use in divorce and parentage cases.  If you do not have an attorney, you are expected to use these forms.  The forms are available on the Supreme Court’s website,  “Criminal, Civil, and Family Forms”.

A number of forms packets for family law cases, including divorce, custody and child support, and parentage are available for purchase from the Court’s Self Help Center ranging from $5 to $20 per packet, or you can download and print them yourself from the different folders below.  The packets include the basic forms you will need for the case.  There are also instructions below for downloading the forms.

The Self Help Center also has many forms for other purposes, such as motions, kinship guardianship, name changes, driver’s license restoration, and others.  If you need a form, ask them first – but remember, they do not have forms for every purpose, and may not have the one you need.  If you do not find the forms you are seeking on our website, you may contact the New Mexico Supreme Court Law Library or an attorney.

The Self Help Center DOES NOT have a Civil Complaint form. Most of the forms at the Self Help Center are free.

The information on this website is not intended as legal advice, and does not substitute for seeking independent legal advice regarding the handling of a lawsuit or related legal matters.

The following forms are available to assist in the legal process at the First Judicial District Court.  If you are unable to locate a specific form or need more information, call 505-455-8146.

Please be aware that the Self Help Center may not have forms for every purpose.  If you do not find the forms you are seeking listed below, you may contact the New Mexico Supreme Court Law Library or an attorney.
